DFA CORE REQUIREMENTS FOR PASSPORT RELEASING | DEPARTMENT OF FOREIGN AFFAIRS

The DFA (Department of Foreign Affairs) in the Philippines has a set of core requirements for passport releasing. These requirements are in place to ensure the security of passport issuance and to maintain the integrity of passport records. By meeting these core requirements, applicants can expedite the passport releasing process and ensure a seamless experience.

  • Receipt issued by DFA
  • Acceptable original identification
  • Renewal of an old passport

If the applicant or his or her authorized representative lost the receipt, an affidavit of loss, notarized by the applicant, must be presented to the bank The affidavit should include a description of the item and the date of purchase, as well as any other relevant information. The bank will require proof of the loss, such as photos or receipts. The bank may also require proof of identity.

In accordance with Department Order No. 2021-012, passports not claimed after one (1) year will be cancelled.

ADDITIONAL REQUIREMENTS

It may be necessary to meet additional requirements. The following section provides more information.

In the event that an applicant cannot claim his/her passport, then a family member is allowed to claim the passport on behalf of the applicant in the case of his/her incapacity to claim one

  • Documentation of affiliation with the applicant (must be included in the authorization letter),
  • The original receipt issued by the DFA,
  • The applicant’s and the representative’s valid photo IDs (originals and photocopies)
  • Members of the immediate family, such as parents, brothers, sisters, spouses, and children who are legal adults

In case someone other than the applicant’s immediate family claims the passport

  • Authorizes the authorized representative with an original notarized Special Power of Attorney (SPA) issued by the applicant
  • Applicants and representatives listed on the Special Power of Attorney must present valid identification (original and photocopy)

In the case of minors

  • Claimant must present original receipt from DFA and valid identification if passport is to be claimed by mother
  • The claiming father must submit the original receipt issued by the DFA, a valid driver’s license, and a PSA document proving parental authority over the minor in order to claim the passport
